Aim                             to measure the quality of hospital discharges (cases / eligible population)

Cases                         a readmission is considered as potentially avoidable if :

- it is related to at least one diagnosis already known during the previous hospital stay
  (exclusion of new affections)

- it was unforeseen at the time of the previous hospital discharge

- it occurs within the 30 days after the previous hospital discharge.

Potentially avoidable readmission in a different hospital is included and is identified by a computerized algorithm: 4 algorithm

Eligible population   The domain is related to acute somatic hospital care units. Following patients are                                    not taken into account:

- patients admitted in psychiatric, geriatric and rehabilitation units

- patients with a rehabilitation or palliative care diagnosis

- patients living in another country

- patients transferred to another hospital
- in-hospital deaths
- healthy newborns
- candidates for one day surgery (4 day surgery) or sleep apnea
- patient discharges during the last month of the survey
